By decree, Government regulates key points contained in health reform

The National Government, through the Ministry of Health, issued Decree 858 of July 30, 2025, with which it regulates some key points contained in the health reform project that is still in process.

Through this regulation, it is reconfigured, immediately, The health services model in the country.

The decree orders “Adopt the preventive, predictive and resolution model as a health policy for the population in the Colombian territory”.

The standard will have application for all health actors, including health or territorial promoters whether departmental, district and municipal; the Superintendence of Health; the EPS; IPS; the administrator of the resources of the General Social Security System (ADRES) and the Social Communities, as well as the other institutions that intervene directly or indirectly, “In the guarantee of the fundamental right to health!.

Among the most prominent points of this decree, it is defined that patients will enter the system through Primary Health Care Centers (CAPS)a figure in which the Government has insisted as a base level of low and medium complexity services.

According to the Government, the basis of this preventive, predictive and resolution model has “As a purpose, to guarantee the cash of the fundamental right to the health of the inhabiting population in Colombian territory, in equal treatment and opportunities, through Plans, programs, projects, strategies and governance oriented and territorial health governance; Comprehensive health care in the integral and integrated health context “.

In addition, it seeks to promote territorial coordination among health actors to strengthen comprehensive care.

The decree also defines a transition period so that the entities and actors of the General System of Social Security in Health progressively adapt their operation to the provisions of the regulations, within a maximum period of up to one year.

“During this period, the actors must adjust their operation to the model of preventive, predictive and resolutive health, their articulation to the integral and integrated territorial health networks – RIITS, and to the new territorial, differential and intersectoral approach defined in this standard”says the norm.

What about EPS?

Under this decree, health promoting entities (EPS) will not disappear but will have substantial changes.

EPS must articulate with the integral and integrated territorial health networks (RIITS) and participate in their organization and operation, in coordination with territorial entities.

This implies a change in its intermediation role, as they will no longer be the only axis of articulation of the system, but an actor within a territorial network guided by the ministry and local authorities.
